How to Register
Registering restrictions = none
We are accepting new patients. If you want to join the practice please complete a registration forms GMS1, which can be picked up from reception. At this point you will be given Health Questionnaires. Please complete and sign the forms before bringing to reception or ask for assistance in completing the form.
The Bring you photo ID and proof of address copy as this will help Practice to give you online access for ordering your repeat medications and appointments once you are fully registered.
For children under 6 years of age, please complete the forms and bring their RED book, if have one otherwise immunization history from previous records, to allow us to check their immunization status.
On receiving the completed forms, we will add you on our computer system. Once the registration process is completed, NHS England will transfer your medical records to us and write to you to confirm your registration as a patient with that practice.
You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
